Hays are actively recruiting for Community Mental Health Nurses/Practitioners in Huddersfield to be part of the Community Mental Health Team, providing holistic care coordination to service users in South Kirklees & Calderdale.
Your new role
As a CPN, you will be responsible for the assessment of needs, planning, implementation, and evaluation of programmes of care for service users, building trusting relationships with them, helping them to adhere to treatment programmes and advising on therapies and lifestyle choices that support good mental health.
