Our client, a Local Authority Yorkshire are currently looking for an enthusiastic social worker to join their Hospital Team. Pay rate will be £35/Hr. Your key responsibilities will include: Our aim is to provide a social work service to adults who have been discharged from an Acute Hospital setting or in some Cases stepped up from the community through Discharge to Assess Team and Intermediate Care Team which provide a similar service but at different bases. The post holder may be required to assess patients/ customers in their own homes and other care home settings. Experience of Intermediate Care, Discharge to Assess social work or hospital discharge social work is essential, as well as the ability to manage your time and prioritise workloads. Knowledge of safeguarding is essential. Actively contribute to maintaining quality standards and ensure self and staff development is maintained.
